Midway Specialty Care Center has adopted the Patient-Centered Medical Home (PCMH) model, which is a new way of delivering healthcare , that promotes a partnership between patients and their healthcare team, providing accessible, coordinated, comprehensive, and continuous quality health care, ensuring that all of their preventive, acute, and chronic care needs are met, emphasizing efficient and timely service delivery.

The medical home responsibilities of Midway Specialty Clinic includes but is not limited to following:
Coordinating patient care across multiple settings.
All patients are expected to be an active participant in their health care by:
Assuming responsibility for certain aspects of their care.
All patients are informed of their medical home responsibilities and that of the Midway Specialty Care Clinic Care Team by way of the following:
Medical Home Brochure which is made accessible to all patients in the reception area and in the exam rooms. This brochure is part of the orientation packet given to all new patients. This information is also made available on Midway Specialty Care Website. A letter outlining the responsibilities of patients and the clinic are given to all new patients.
