All new patients should complete the forms and keep the reference document for your files. Save time before your first visit, by filling out paperwork at home. When you have completed them, please sign, and bring them to your appointment at Midway Specialty Care.

The mission of Midway Specialty Care Center, as a not-for-profit clinic, is to provide our patients a comprehensive infectious disease care center, with primary care, and access to behavioral health services, gynecological care and nutritional support. Our dedicated team of physicians, physician assistants, nurse practitioners, nurses, medical assistants, and pharmacists work to provide total patient care with dignity to those living with an infectious disease.
We accept most commercial insurances. Please call the office in advance to verify if we take your insurance. We also accept Medicaid and Medicare. If you do not have insurance, ask for a case manager to work with you on a self-payment option, using a sliding scale based on your household income and family size.
Midway Specialty Care Center is proud to partner with Dr. Michael Dunn at our Tampa office. He is a certified member of the American Association of HIV Medicine and Fellowship Trained in Infectious Diseases Dr. Dunn specializes in Internal Medicine and Infectious Diseases, with an emphasis on HIV and Hepatitis.
Dr. Dunn has been an integral part of the HIV care in the Tampa Bay area for over 20 years and is highly respected in his field. Dr. Dunn received his medical degree from University of Tennessee College of Medicine.
Dr. Dunn is honored as a winner of the 2018 Tampa Bay Business Journal Business of Pride award. The companies, organizations and individuals honored are working to advance lesbian, gay, bisexual and transgender equality in the workplace. Read more
Rodrigo Sabec is originally from Brazil but relocated and settled in the Tampa area. He received his Master’s degree in Sciences of Nursing from South University in Tampa, Florida, and he is Board Certified by The American Academy of Nurse Practitioners National Certification Board, Inc. as a Family Nurse Practitioner.
He has a focused interest in HIV treatment, transgender care and PrEP and enjoys spending time with Ginger and Ella (his dogs) and loved ones.

It is the policy of Midway Specialty Care Center to provide a safe environment for patients and employees. The wearing of masks is an integral factor in preventing the spread of respiratory infections.
Patients/clients will be asked to wear a mask upon arrival at any Midway facility under the following circumstances:
Midway reserves the right to refuse accommodation for patients who exhibit any signs and or symptoms of respiratory infection and refuse to wear a mask when asked to do so.
All employees are encouraged to wear masks when in close contact with patients/clients.
