All new patients should complete the forms and keep the reference document for your files. Save time before your first visit, by filling out paperwork at home. When you have completed them, please sign, and bring them to your appointment at Midway Specialty Care.

The mission of Midway Specialty Care Center, as a not-for-profit clinic, is to provide our patients a comprehensive infectious disease care center, with primary care, and access to behavioral health services, gynecological care and nutritional support. Our dedicated team of physicians, physician assistants, nurse practitioners, nurses, medical assistants, and pharmacists work to provide total patient care with dignity to those living with an infectious disease.
We accept most commercial insurances. Please call the office in advance to verify if we take your insurance. We also accept Medicaid and Medicare. If you do not have insurance, ask for a case manager to work with you on a self-payment option, using a sliding scale based on your household income and family size.
Dr. Scotto D’Antuoro received his Medical Degree from Ross University School of Medicine in New York. He then completed an Internship at Hahneman University
Hospital in Philadelphia, Pennsylvania followed by a Residency Program in Internal Medicine at Robert Wood Johnson University Hospital in New Brunswick,
New Jersey. Dr. Scotto D’Antuono realized that he was passionate about Infectious Diseases and continued his training by completing a Fellowship Program in Infectious Diseases at Robert Wood University Hospital in New Jersey.
Dr. Scotto D’Antuono was in private practice in the northeast before relocating to Florida with his wife and family. Dr. Scotto D’Antuono speaks Spanish as well as Italian, loves animals and spending time with his family.
Terry Law, APRN has over 20 years of clinical experience. His areas of expertise include HIV and HCV management, clinical trials and medication studies, preventative medicine, medical pharmacology education lectures at a local Tampa University. He is an active member of the American Association of Nurse Practitioners, American Academy of HIV Medicine, serves on the Nurse Practitioner Committee and the Transgender committee. Terry also serves on the Ryan White Health Services Advisory Committee presenting Central West Florida.
Terry earned his Associates degree of Applied Sciences in Nursing in 2005 at Wake Technical Community College in Raleigh, North Carolina. In 2013, Terry completed his Bachelor of Science-Master of Science in Nursing (BSN-MSN) Family Nurse Practitioner degree from the University of North Carolina at Chapel Hill, Chapel Hill, North Carolina.
Prior to follow his passion in medicine, Terry earned his Bachelor of Arts in French Translation after living in France and studying at L’Institut Catholiqe in Paris. He remained in Paris as a personal cook for a family prior to returning the US in 1991. Terry enjoys traveling, retiring from United Airlines (formerly Continental Airlines) after serving 21 years an International Flight Attendant and French Interpreter. Terry enjoys playing the violin, viola and French horn and has played with the local Tampa Pride band. He enjoys cooking and spending time with his two dogs, Grayson and Jazz.

It is the policy of Midway Specialty Care Center to provide a safe environment for patients and employees. The wearing of masks is an integral factor in preventing the spread of respiratory infections.
Patients/clients will be asked to wear a mask upon arrival at any Midway facility under the following circumstances:
Midway reserves the right to refuse accommodation for patients who exhibit any signs and or symptoms of respiratory infection and refuse to wear a mask when asked to do so.
All employees are encouraged to wear masks when in close contact with patients/clients.
